Up to 80% of adults experience back pain at some point. For about one in five, it becomes chronic — lasting months or years, limiting daily activity, and resisting simple fixes. Chronic low back pain involves a tangle of factors: muscle tension, spinal joint inflammation, nerve irritation, and the stress-pain feedback loop where guarding against pain creates more tension.

Conventional management relies on physical therapy, anti-inflammatory drugs, exercise, and — in severe cases — spinal injections. Peat therapy enters the picture as a complementary approach that addresses several of these factors simultaneously.

Why peat may help

Back pain responds well to heat. But peat delivers heat differently than a hot water bottle or a wheat bag.

Heat that lasts. Peat’s exceptional thermal retention means a peat pack or bath maintains therapeutic temperature for the entire treatment session. The muscles along your spine — the paravertebral muscles — need sustained warmth to truly relax. A peat mud pack on the lower back holds its heat long enough for deep muscle tissue to release tension, something a conventional hot pack can’t match because it cools too quickly.

Anti-inflammatory chemistry on top of heat. While the heat relaxes muscles, humic acids absorbed through the skin work on the inflammatory component. Spinal joint inflammation — particularly in facet joints — is a significant driver of chronic back pain. The anti-inflammatory compounds in peat suppress the same inflammatory signals (TNF-α, IL-6) involved in joint degradation, complementing the purely thermal effect.

The immersion effect. In a full peat bath, buoyancy partially unloads the spine while warmth and chemical absorption work together. The sensory experience also interrupts the stress-guarding-pain cycle that keeps back muscles locked.

What the evidence says

The evidence for peat in back pain is moderate — not as strong as for joint inflammation, but supported by systematic reviews and randomised trials.

A 2025 systematic review covering the past five years of research found consistently positive outcomes for balneotherapy in chronic low back pain. An RCT by Karaarslan (2022) compared mud-pack therapy to hot-pack therapy for chronic spinal pain and found mud packs produced significantly better outcomes — again confirming that peat adds something beyond heat alone. A single-blind RCT targeting chronic low back pain specifically (Kardes 2021, n=106) found peloid therapy added to a home-exercise programme significantly outperformed home exercise alone on pain, disability, and vitality scores — direct evidence that peat therapy adds value on top of the standard conservative approach, not just as a substitute for it.

The broader balneotherapy literature, including the strong evidence for musculoskeletal conditions generally (Beer 2013), provides additional support.

What’s missing: Back-pain-specific RCTs isolating peat therapy from other spa interventions. Most studies come from rehabilitation settings where peat is one component of a broader programme. Separating the peat effect from exercise, rest, and stress reduction is methodologically difficult.

Peat therapy won't fix structural spinal problems, but for the muscle tension, joint inflammation, and stress components of chronic back pain, the evidence supports it as a useful complement to physical therapy and exercise.
