From In Vitro to Clinical

Both levels of evidence are needed — in vitro data shows mechanism, clinical data shows whether the mechanism operates at therapeutically relevant scale in a living system.

T-Cell and Cytokine Modulation

Balneotherapy shifts T-cell population ratios and cytokine profiles toward regulatory states. Specifically, studies document reductions in pro-inflammatory T-helper-1 (Th1) and Th17 activity, with relatively preserved or enhanced regulatory T-cell (Treg) function. At the cytokine level, TNF-α, IL-1β, and IL-6 decrease while IL-10 (anti-inflammatory) is maintained or increased. This profile is clinically meaningful: it resembles the target of biological drugs used in autoimmune conditions, achieved here without systemic immunosuppression.

The Anti-Inflammatory vs. Immunomodulatory Distinction

The review explicitly addresses this distinction. Anti-inflammatory means suppressing inflammation — broadly reducing immune activity. Immunomodulatory means rebalancing immune regulation — normalising an over-activated response while preserving functional immunity. Balneotherapy appears to act immunomodulatorily rather than immunosuppressively: patients do not show increased infection rates after treatment, which would be expected if the immune system were simply suppressed.
