A well-designed randomized controlled trial published in Rheumatology International, comparing peloid therapy directly to hot-pack therapy for knee osteoarthritis. With 60 patients, a proper control group, and a 3-month follow-up, this is among the stronger individual RCTs supporting peloid therapy in musculoskeletal conditions.

Study Design

60 patients with knee osteoarthritis were randomized to two groups: peloid therapy or hot-pack control. Treatment duration was 2 weeks, daily sessions. Both groups received the same heat stimulus — the only difference was the presence or absence of the peloid’s chemical content. This comparison is the correct scientific approach to isolating whether peat adds anything beyond heat.

What Was Measured

The primary outcome was WOMAC (Western Ontario and McMaster Universities Arthritis Index) — the standard validated questionnaire for knee OA measuring pain, stiffness, and functional limitation. Follow-up assessments were conducted at the end of treatment and at 3 months.

Key Findings

The peloid group showed significantly greater improvement in WOMAC pain, stiffness, and function scores compared to the hot-pack control. More importantly, the improvements were sustained at 3-month follow-up in the peloid group. A hot-pack’s effect would be expected to diminish after treatment ends as the thermal stimulus disappears. The sustained benefit points to a lasting biological effect from the chemical components of the peloid — consistent with the anti-inflammatory and tissue-modulating mechanisms attributed to humic acids.

Significance

This trial directly addresses the core scientific question: is peat therapy just expensive hot water? The maintained 3-month outcomes suggest it is not. It is one of the key trials cited when the evidence for peloid therapy in knee osteoarthritis is graded as “strong.”
